Job summary

Inizio Engage is recruiting a community-based Registered Nurse to deliver safe, patient-centred PN and IV therapies in patients' homes across Kent and surrounding areas. Travel between visits is part of the role, with 40 hours per week from Monday to Sunday.

The successful candidate will assess, educate and support patients and carers, collaborating with NHS teams and clinical mentors to ensure seamless transitions from hospital to home.

Qualifications

• Current NMC Registration is essential.
• A degree-level qualification or equivalent experience is required.
• Minimum one year post-registration nursing experience is expected.
• Evidence of continuing professional development (CPD) is desirable.
• Current valid UK driving licence is required.
• Experience in CVAD and/or Parenteral Nutrition services is advantageous.

Responsibilities

• Deliver safe, high-quality, patient-centred care in patients' homes and community settings.
• Administer prescribed Parenteral Nutrition (PN), IV therapies and clinical interventions per SOPs and standards.
• Assess patients, identify changes in condition and escalate clinical concerns accordingly.
• Support treatment initiation, ongoing care and transitions from hospital to home.
• Train and empower patients, families and carers to manage treatment safely and independently.
• Provide clinical advice and build strong relationships with patients and healthcare professionals.
• Collaborate with NHS colleagues and senior homecare/clinical teams to ensure continuity of care.
• Maintain accurate clinical records and follow governance and policy standards.
• Support safety, quality improvement and KPIs within the service.
