Job summary

Hampshire and Isle of Wight Healthcare NHS Foundation Trust is recruiting a part-time Community Healthcare Support Worker to join our community nursing team. The role involves delivering high-quality care to patients in their homes and will be supported by a registered nurse.

You will undertake a range of clinical tasks, with training provided, and contribute to the efficient running of the service. The post requires a full UK driving licence, access to a vehicle, and excellent communication

Qualifications

• GCSE in Maths and English or equivalent.
• Full UK driving licence and access to a vehicle for work use.
• Excellent communication skills to engage with patients and manage complex needs.

Responsibilities

• Assist qualified staff in community-based care delivery.
• Undertake phlebotomy for patients in their own homes.
• Provide diabetic care including blood glucose monitoring and insulin administration.
• Undertake wound and leg ulcer care.
• Catheter care and re-catheterisation.
• Undertake basic observations such as blood pressure and report to team lead/GP.
• Obtain and process a variety of specimens.
