Hampshire and Isle of Wight Healthcare NHS Foundation Trust is seeking a Band 5 Community Nurse to deliver compassionate, evidence-based care at home, in care homes, and other community settings. You will work autonomously within a supportive team, using clinical judgment to develop and review care plans while maintaining high patient experience.

The role requires a UK driving licence with access to a vehicle, and you will work a range of early/late shifts across weekdays, weekends and bank

Qualifications

• UK nursing degree and current registration with NMC.
• Willingness to travel to people’s homes and care settings.
• Commitment to evidence-based practice and safeguarding.

Responsabilités

• Holistic patient assessment, formulation and review of care plans.
• Wound management including pressure ulcers.
• Palliative and end-of-life care.
• Bladder and bowel care including catheterisation.
• Medicines management, injections and phlebotomy.
• Provision of equipment to support care.
• Safeguarding of adults and line management of junior staff.
• Supervision of nursing students and ongoing professional development.
