Job summary

East Lancashire Hospitals NHS Trust is seeking a dedicated Integrated Discharge Service coordinator to support the Pennine Lancashire Home First pathway. You will manage the Hub, process referrals, and ensure safe, timely discharges across hospital to home.

Working across a seven-day service (including bank holidays) with flexible hours (8am–5pm), you will foster clear communication between wards, community teams and other services, supporting a responsive discharge process.

Responsibilities

• Manage the Integrated Discharge Service Hub and Home First referrals.
• Coordinate safe, timely discharges from hospital to home across Pennine Lancashire.
• Maintain accurate records and participate in audits for discharge planning.
• Escalate issues to the Senior Manager or Site Manager when delays occur.
• Promote effective communication between wards, community teams and other services.
