East Lancashire Hospitals NHS Trust is seeking a dedicated Integrated Discharge Service coordinator to support the Pennine Lancashire Home First pathway. You will manage the Hub, process referrals, and ensure safe, timely discharges across hospital to home.
Working across a seven-day service (including bank holidays) with flexible hours (8am–5pm), you will foster clear communication between wards, community teams and other services, supporting a responsive discharge process.
This role is an integral part of the Home First pathway supporting patients across the Pennine Lancashire footprint. The role will work alongside the Pennine Lancashire Home First pathways co-ordinating the safe transport of patients from hospital to home. The role will also include the management of the IDS Hub and the Home First referrals and supporting wider teams The service operates 7 days per week including bank holiday and hours will be flexible 8am – 5pm.
Work flexibly across the entire Pennine Lancashire Home First pathway to meet the needs of all Home First discharges for all adult patients being discharged or requiring community support.
Drive communication and response between relevant services through Hub management and Home First referrals
To ensure you are an integral part of a responsive team by delivering a highly responsive and effective service delivery increasing utilisation of the dedicated Home First Transport.
Complete a daily work allocation and ensure completion of all tasks to effectively operate the service, within this demonstrate a flexible approach to meet individual patients/family need.
Report & monitor problems with transport services and esclate issues to senior clinical flow team.
To promote and maintain effective communication channels between relevant teams in order to successfully coordinate discharges.
Proactively advise and support staff to initiate the early discharge planning. Encourage wards to utilise the Discharge Lounge for Home First discharges
Identify, progress chase and monitor delayed discharges highlight to Senior Manager or Site Manager
Maintain accurate records and participate in audits as required in relation to the Pennine Lancashire Home First discharge planning process.
Support and respond to the Trust Escalation Processes
Ensure compliance with other related Trust Policies and Department of Health Legislation with regard to discharge planning processes.
You will work as part of the Integrated Discharge Service. You will be responsible for the day to day management of the Hub, Home First referrals and ensuring safe effective discharges proceed without delay.
You will support the wider team and have a good understanding or a commitment to learn the discharge processes.
