Job summary

Blackpool Council is recruiting a Transfer of Care Co-ordinator to join the Home Care and Reablement Service. You will be the operational link between hospital and community care, coordinating referrals and ensuring timely discharge planning.

In this fast-paced role, you will lead daily service huddles, deploy Home Care staff via electronic rosters, and collaborate with multidisciplinary teams to deliver high-quality, safe care in people’s homes.

Qualifications

• A Level 2 qualification in Health & Social Care (or equivalent).
• Experience working in health and social care environments.

Responsibilities

• Coordinate transfers of care from hospital and community settings.
• Lead daily service huddles to identify priorities and manage demand.
• Allocate and deploy Home Care staff via electronic rota systems.
• Work closely with the Transfer of Care Hub and multidisciplinary partners.
• Monitor service capacity and identify solutions to meet demand.
• Support service performance reporting and funding data requirements.
• Provide coaching and operational support to administration colleagues.
• Liaise with service users, families, care providers and professionals.
• Ensure timely, high-quality support for service users.
