HomeLink Healthcare is seeking an experienced Registered Nurse to join the Kettering team as a Discharge Liaison Nurse, coordinating safe transfers from hospital into community services.
You will combine clinical assessment with patient coordination, working with hospital teams, families and multidisciplinary colleagues to support timely discharges and ongoing care at home.
Qualifications
- Active NMC registration is required.
- Minimum of three years' post-registration nursing experience.
- Experience caring for adults with diverse clinical conditions.
- Strong clinical assessment and decision-making skills.
- Excellent communication and multidisciplinary teamwork abilities.
Responsibilities
- Work with hospital teams to transfer patients to community services.
- Undertake holistic clinical assessments for suitability of community care.
- Assess pathways including virtual ward and early supported discharge.
- Review medical history and ongoing care requirements.
- Identify discharge barriers and coordinate with MDT.
- Plan discharge and transfer arrangements before community move.
- Liaise with Trust and HomeLink Healthcare teams for handovers.
- Provide information and support to ward teams and families.
- Support patients and families in care decisions and options.
- Undertake autonomous clinical duties within scope of practice.
- Support Clinical Lead and team, deputise when needed, participate in on-call.

HomeLink Healthcare is looking for an experienced Registered Nurse to join our Kettering team as a Discharge liason nurse, supporting the safe and timely transition of patients from hospital into our Hospital at Home and community services.
This is an ideal opportunity for an experienced nurse who enjoys combining clinical assessment with patient coordination and multidisciplinary working.
As a Discharge Liaison Nurse, you will work closely with hospital teams, patients, families and HomeLink Healthcare colleagues to assess patients for our services and coordinate safe transfers of care into the community.

As a Discharge liason nurse, you will play an important role in supporting patient flow and ensuring patients can move safely and effectively from hospital into appropriate community-based care.
You will:
- Work closely with hospital teams to facilitate the safe and timely transfer of patients from hospital into HomeLink Healthcare's community services
- Undertake holistic clinical assessments to determine patients' suitability for community-based care
- Assess patients for pathways including virtual ward and early supported discharge services
- Review patients' medical history, current clinical needs and ongoing care requirements
- Identify potential barriers to discharge and work collaboratively with the wider multidisciplinary team to resolve them
- Coordinate discharge and transfer plans, ensuring appropriate arrangements are in place before patients move into the community
- Liaise closely with Trust multidisciplinary teams and HomeLink Healthcare clinical teams
- Complete and document clinical handovers accurately and in a timely manner
- Provide advice, information and support to ward teams and other healthcare professionals
- Support patients and families to understand their care options and make informed decisions about their ongoing care
- Undertake clinical duties as an autonomous practitioner within your scope of practice
- Support the Clinical Lead and wider team with the safe and effective delivery of the service
- Deputise for the Clinical Lead where required
- Participate in the on-call rota where required to support service continuity
- Contract: Permanent, Monday to Friday
You will be an experienced Registered Nurse who is confident undertaking clinical assessments and making informed decisions about patients' ongoing care.
You'll be comfortable working autonomously while also collaborating closely with hospital teams, multidisciplinary colleagues, patients and families.
Compassionate and organised, you'll be able to manage competing priorities while maintaining a strong focus on patient safety, communication and continuity of care.
You will also need:
- Active NMC registration
- A minimum of three years' post-registration nursing experience
- Recent experience caring for adults with a broad range of clinical conditions
- Strong clinical assessment and decision-making skills
- Excellent communication and multidisciplinary working skills
- Confidence working autonomously across hospital and community settings
- Understanding of safe discharge planning and transfer of care
- Knowledge of the Care Act 2014 and relevant legislation relating to hospital discharge
- Full UK driving licence and access to your own vehicle
- Right to work in the UK (sponsorship is not available)
- Previous experience in discharge planning, patient flow, community services or Hospital at Home
- Experience of virtual wards or early supported discharge pathways
- Previous management, leadership or staff supervision experience
