HomeLink Healthcare is looking for an experienced Registered Nurse to join our Kettering team as a Discharge liason nurse, supporting the safe and timely transition of patients from hospital into our Hospital at Home and community services.
This is an ideal opportunity for an experienced nurse who enjoys combining clinical assessment with patient coordination and multidisciplinary working.
As a Discharge Liaison Nurse, you will work closely with hospital teams, patients, families and HomeLink Healthcare colleagues to assess patients for our services and coordinate safe transfers of care into the community.
As a Discharge liason nurse, you will play an important role in supporting patient flow and ensuring patients can move safely and effectively from hospital into appropriate community-based care.
You will:
You will be an experienced Registered Nurse who is confident undertaking clinical assessments and making informed decisions about patients' ongoing care.
You'll be comfortable working autonomously while also collaborating closely with hospital teams, multidisciplinary colleagues, patients and families.
Compassionate and organised, you'll be able to manage competing priorities while maintaining a strong focus on patient safety, communication and continuity of care.
