Job summary

Pennine Care NHS Foundation Trust in Greater Manchester seeks a Band 5 Community Learning Disability Nurse. You will join a multidisciplinary team delivering person-centred care for people with learning disabilities and complex needs in the community.

You will manage your own caseload, undertake specialist assessments and work across pathways including Positive Behaviour Support, Autism, Epilepsy, Dysphagia and Dementia.

As a Band 5 Community Learning Disability Nurse, you will work as part of a supportive multidisciplinary team, delivering specialist pathways, evidence-based interventions and person-centred care to individuals with learning disabilities and complex needs

Main duties of the job

Managing your own caseload within the community.

Undertaking specialist health and behavioural pathways, assessments.

Supporting individuals in a range of settings, including their own homes.

Promoting health, wellbeing and reducing health inequalities for people with learning disabilities.

Working closely with families, carers, primary care, social care and partner agencies.

Contributing to specialist pathways including Positive Behaviour Support, Physical Health, Autism, Epilepsy, Dysphagia and Dementia.
