Job summary

Gloucestershire Health and Care NHS Foundation Trust is seeking a motivated Band 4 professional for the Home First service. The role supports people recently discharged from hospital to maximise independence and works with health and social care services using a case management approach.

The Neighbourhood Connector will act as a single point of contact and build personalised relationships to enable people to live their best life possible.

Qualifications

• NVQ Level 3 in health and social care management or similar qualification.
• GCSEs English & Maths Grade C or above.
• Health Coaching qualification.

Responsibilities

• The post holder will visit individuals at home to undertake personal assessments and ensure support to live the life they wish.
• Establish and maintain excellent communication with individuals and explore complex care options and decisions.
• Sustain effective working relationships across all health and social care services.
• Support a defined group of individuals to promote independence through case management/self-care approaches.
