Kent Community Health NHS Foundation Trust is seeking a Senior Assessor to deliver high-quality, person-centred care in Home First. You will provide advanced assessment, complex case management and operational support, guiding and developing colleagues to maintain team excellence.

The role includes leading on complex discharge planning, supervising Band 4 staff and supporting Band 5 assessors, with responsibilities for rota planning and service delivery.

Qualifications

• Advanced clinical assessment and holistic case management.
• Experience leading Band 4 staff and supervising Band 5 assessors.
• Knowledge of safeguarding procedures and quality improvement.

Responsabilités

• Undertake timely, holistic assessments of patients’ needs.
• Lead complex discharge planning ensuring safe transitions home.
• Provide day-to-day leadership, guidance and support for Band 4 staff.
• Oversee training and competency development of Band 5 assessors.
• Coordinate rota planning, tasks and daily workforce dynamics.
• Monitor workloads and escalate service pressures to maintain safe staffing.
