Job summary

Gloucestershire Health and Care NHS Foundation Trust in Gloucestershire is seeking Home First Assistants to support people at home after hospital discharge. Roles cover East Gloucestershire, including Gloucester, with shifts 7am–10pm on a 4-on, 4-off pattern.

Permanent contracts, full-time or part-time across 36.75 hours per week. You will perform holistic assessments, provide reablement and personal care, collaborate with therapists and nurses, and maintain accurate records to help people

Qualifications

• GCSE Grade C/4 or equivalent in English and Maths.
• Level 3 Senior Healthcare Support Worker Apprenticeship, NVQ Level 3, or equivalent Health and social care qualification.
• Care Certificate.

Responsibilities

• Support adults in their own homes following hospital discharge, helping regain independence and daily living skills.
• Undertake holistic and risk assessments, support daily living activities, and monitor progress.
• Support people to use equipment safely, promote wellbeing and independence, and engage with community resources.
