Job summary

Qualifications

• Registered Nurse (Adult) with current NMC registration.
• Experience delivering holistic nursing assessments.
• Ability to work autonomously within a multidisciplinary team.
• Knowledge of safeguarding procedures.

Responsibilities

• Complete holistic nursing assessments and develop personalised care plans for patients discharged through Home First.
• Support timely hospital discharges and prevent avoidable admissions.
• Deliver nursing care for patients with complex and long-term conditions.
• Coordinate handover to Community Nursing Teams as required.
• Promote independence through education and health coaching.
