Job summary

Leaf Complex Care is seeking a Healthcare Assistant to provide care in people’s homes and communities in the Bath area. You will support individuals with complex needs, including autism, in personal care, household tasks, budgeting, and planning community activities.

Responsibilities include adapting care plans, maintaining records, and communicating with families and the care team. Right to work in the UK is required; six months’ relevant experience preferred.

Qualifications

• 6 months' experience in healthcare settings.
• Experience supporting people with complex needs, including autism.

Responsibilities

• Deliver person-centred care in clients' homes and communities
• Assist with personal care including hygiene, dressing and feeding
• Support people to maintain or develop skills and interests
• Adapt care plans to individual needs, ensuring comfort and safety
• Support people in temporary residential placements or their own homes
• Assist with budgeting and financial management
• Plan and participate in community activities within budget
• Carry out household tasks such as laundry, cleaning and shopping
• Facilitate social, educational or work activities per choice
• Follow care plans and report changes to the Registered Manager or Care Coordinator
• Complete daily records on the digital care planning system
• Communicate proactively with families and other healthcare professionals
