Job summary

The Leaf Complex Care Limited, part of the Catalyst Care Group, is seeking a Healthcare Assistant to provide person-centred care in clients' homes and communities. You will assist with personal care, household tasks, budgeting support, and help clients engage with community activities, adapting plans to each individual's needs.

Shifts are flexible and you’ll work within multi-disciplinary teams, keeping accurate records on the digital care planning system and reporting changes to the Registered

Qualifications

• Minimum of 6 months' experience in similar healthcare settings.
• Experience supporting people with complex needs, behaviours that challenge and autism.
• Right to work in the UK

Responsibilities

• Deliver person-centred care in people’s homes and communities.
• Adapt care and support plans to each person’s needs, comfort and safety.
• Assist with personal care including hygiene, dressing and feeding.
• Support people in temporary residential placements or their own homes.
• Support people to maintain or develop skills, interests and community connections.
• Plan, take part in and budget for community and social activities.
• Support people with budgeting and financial management.
• Carry out household tasks such as laundry, cleaning and food shopping.
• Follow care plans and report significant changes in need to the Registered Manager or Care Coordinator.
• Keep accurate daily records on the digital care planning system.
• Communicate with families, care teams and other healthcare professionals.
• Work with multi-disciplinary teams and promote and safeguard people’s welfare.
