Job summary

Bluebird Care is seeking a proactive Care Coordinator to help grow the team, support colleagues and deliver person-centred care in clients’ homes. You will create rotas, coordinate staff, and ensure care plans are followed with clear communication across the team.

You will support on-call duties, maintain records and build strong relationships with customers, carers and external professionals, emphasising confidentiality and high standards of service.

Qualifications

• Experience in scheduling or care coordination is beneficial but not essential.
• Excellent organisational and communication skills are required.

Responsibilities

• Create and maintain efficient rotas for timely care delivery.
• Coordinate staffing levels and skill mix with the Registered Manager.
• Arrange cover for staff holidays and absences.
• Process referrals and changes to care needs promptly.
• Support Care Experts by monitoring workloads and promoting work-life balance.
• Match Care Experts to customers based on skills, experience, availability and preferences.
• Participate in on-call duties following training and competency sign-off.
• Maintain accurate records, including travel and incident reporting.
• Ensure confidentiality and adherence to company policies.
