Job summary

Sugarman Health & Wellbeing is seeking a Care Coordinator to organise and deliver personalised care for service users in their own homes and in the community. Qualifications

• DBS check is required for this role.
• Experience coordinating care and resources for home-based clients.
• Ability to work with multidisciplinary teams and families.

Responsibilities

• Ensure sufficient workers to meet client needs.
• Coordinate with Registered Manager and care teams on policy updates.
• Monitor health and wellbeing changes and communicate with stakeholders.
• Ensure workers comply with CQC standards and company procedures.
• Maintain worker records in line with policies and procedures.
• Resolve queries from clients and families in a timely manner.
• Liaise with Commissioners, service users and government bodies as needed.
• Support safe working practices and remote workforce management.
