Sugarman Health & Wellbeing Division is seeking a Care Coordinator to provide and coordinate individualised care and resources for service users in their homes and community settings. You will work with multidisciplinary teams to ensure high standards of care, maintain records, and manage essential relationships with families and government bodies.

DBS clearance is required. The role emphasises exceptional customer service, adherence to policy, and alignment with the company’s long-term vision

Qualifications

• Experience coordinating care in home or community settings.
• Ability to maintain records in line with policy.
• DBS clearance is required for this role.

Responsabilités

• Ensure enough workers are available to meet client needs.
• Stay up to date with legislation and company policy with managers.
• Monitor health and wellbeing changes and communicate with multidisciplinary teams.
• Ensure workers are compliant with CQC standards and group procedures.
• Maintain worker records in line with procedures and standards.
• Communicate changes to families and government bodies as needed.
• Manage relationships between service users, families and government bodies.
