Sugarman Health & Wellbeing Division is seeking a Care Coordinator to provide and coordinate individualized care and resources for service users in their own homes and in the community. You will work with multidisciplinary teams, maintain compliant worker records, and support families and service users while upholding safeguarding and DBS requirements.

You will ensure safe working practices and liaise with management to meet client needs.

Qualifications

• DBS check is required.
• Experience coordinating care in home or community settings.
• Knowledge of safeguarding and relevant legislation.

Responsabilités

• Coordinate individualized care and resources for service users in their own homes and community.
• Liaise with multidisciplinary teams including Social Workers, District Nurses and CCGs.
• Maintain worker records in line with policy and procedure and assist with compliance.
• Communicate changes in service user condition to families and government bodies as required.
• Ensure safe working practices and escalation of business, operational or legal needs to management.
• Support and supervise care workers within assigned area in collaboration with branch management.
