Sugarman Health & Wellbeing division seeks a Care Coordinator to plan and coordinate care for service users in their homes and communities. You will manage resources, work with multidisciplinary teams, and ensure care aligns with policy while delivering the company's long-term goals.

Responsibilities include ensuring adequate staffing, staying updated on legislation, and maintaining compliant records. DBS is required; progression to Senior Care Coordinator is possible.

• DBS required for this role.

Responsabilidades

• Ensure sufficient workers to meet client needs.
• Stay up to date with changes in relevant legislation and policy.
• Monitor health and wellbeing of service users and communicate changes to teams.
• Ensure workers are compliant with CQC standards and group procedures.
• Maintain worker records and keep documents up to date.
• Resolve mobile work queries and address client questions.
