Job summary

Sugarman Health & Wellbeing Division is seeking a Care Coordinator to provide and coordinate personalised care for service users in their homes and community settings. You will manage resources, work with multidisciplinary teams, and ensure care complies with policy and procedure while delivering excellent customer service.

Qualifications

• DBS check is required for the role.
• Experience coordinating care teams in community/home settings.
• Ability to manage relationships with service users and families.

Responsibilities

• Coordinate individualised care and resources for service users.
• Manage and distribute resources effectively in the community.
• Liaise with Social Workers, Nurses and families to plan care.
• Ensure care complies with policy, procedures and safety standards.
• Maintain worker records and ensure DBS and regulatory compliance.
