Job summary

Qualifications

• Experience in rehabilitation or community health settings.
• Knowledge of discharge to assess (D2A) principles and pathways.
• Ability to work in patients' homes and within multidisciplinary teams.

Responsibilities

• Provide rehabilitation and reablement to patients immediately after hospital discharge.
• Support short-term assessment, recovery and rehabilitation in the patient home.
• Assist with home-based assessments to determine future care needs.
• Hours are 08:00–20:00, 7 days a week; part-time working can be considered.
