Job summary
Belfast Health & Social Care Trust is seeking a Rehabilitation Assistant for the Community Stroke Team to deliver intensive rehabilitation in clients' homes, under therapist supervision. The role supports daily living activities and promotes independence with a patient-centred approach.
The post requires flexible working between 8 am and 10 pm, with a roster including weekends and bank holidays, in a community setting across Belfast.
Qualifications
- Hold NVQ/VRQ/QCF/RQF level 2 Certificate in Health/Health & Social Care (or equivalent) within 12 months of taking up post.
- For Community post: must hold a full driver's licence and have access to a car or transport to carry out duties.
- Must undertake regional induction in first week and complete within 6 months.
- Must undertake relevant training to meet service needs.
Responsibilities
- Provide care at clients' homes and assist with activities of daily living as delegated by nursing staff.
- Maintain confidentiality of patient information at all times.
- Record activities accurately and communicate changes to therapists/line manager.
- Assist in care plans and participate in client review meetings.
- Work flexibly between 8 am and 10 pm; shifts include 8am-4pm or 2pm-10pm; weekend/bank holidays rostered.
