Job summary

East London NHS Foundation Trust in London seeks a confident Rehabilitation Support Worker to assist physiotherapists and occupational therapists with home-based rehabilitation for patients in Tower Hamlets Community Health Teams.

You will implement rehab plans, involve clients and carers in care planning, and support self-management while contributing to multidisciplinary team discussions and patient progress reporting.

Qualifications

• Ability to undertake manual handling activities.
• Knowledge and experience of rehabilitation/therapy skills and activities.
• Ability to work as part of a team and as a lone worker.

Responsibilities

• Contribute to provision of rehabilitation programs under supervision of therapists.
• Assist in planning care/rehabilitation programs with clients and carers.
• Participate in goal planning and report progress to therapists.
• Collaborate with nursing and therapy staff within the MDT.
• Develop rehabilitation skills while working with clients with physical disability.
