Job summary

EAST LONDON NHS FOUNDATION TRUST is seeking a confident rehabilitation support worker to assist physiotherapists and occupational therapists in delivering treatments to patients in their homes and community settings.

You will carry out rehab programs, help clients and carers plan care, and promote self-management to maximise mobility and independence.

Qualifications

• Implement rehabilitation plans written by clinicians.
• Involve clients and carers in planning and encourage self-management.
• Contribute to goal planning and act on senior staff guidance.

Job description

An exciting opportunity has arisen for a confident proactive individual within Tower Hamlets Community Health Teams. A rehabilitation support worker is required to assist physiotherapists/occupational therapists in providing treatment to patients. The post holder will be responsible for carrying out rehabilitation programs with patients, as devised by the physiotherapist/occupational therapist, to maximise patient's mobility and independence in daily activities in their own home.,

• Responsible for implementing and supporting rehab plans written by Physiotherapists and Occupational Therapists.
• To involve clients and carers in the planning of care/rehabilitation programmes and encourage self-management where appropriate.
• Contributes to client goal planning sessions and shows initiative in areas where competent and as agreed by senior staff.
