Job summary

Civic Recruitment Limited is seeking an Occupational Therapist for the Reablement at Home Service in Hackney. The role involves delivering specialist OT interventions, conducting assessments, and developing therapy-led reablement plans to help adults regain independence after hospital discharge.

You will work with carers, reablement providers and multidisciplinary teams to promote person-centred goals and reduce dependency on long-term care. Hackney location, 5-month contract.

Qualifications

• Must hold a Degree in Occupational Therapy.
• HCPC registration is required.
• Enhanced DBS clearance is required.
• Experience in Adult Social Care or Community Services beneficial.
• Experience in reablement, rehabilitation or discharge-to-assess services.
• Strong report writing, assessment and analytical skills.

Responsibilities

• Undertake comprehensive Occupational Therapy assessments for adults with complex needs.
• Develop personalised Reablement Therapy Plans focused on outcomes.
• Support individuals to regain skills for independent living.
• Promote a strengths-based, preventative approach to improve quality of life.
• Support hospital discharge pathways and reduce long-term care demand.
• Supervise OT Assistants and support students/junior staff.
• Collaborate with multi-disciplinary teams and reablement providers.
