Job summary

One Wight Health Ltd on the Isle of Wight is seeking a full-time Social Prescribing link worker to join the Islands South Primary Care Network at the Bay Medical Practice (Sandown and Shanklin). The role is permanent, 37.5 hours per week, with a salary of £31,150 per annum.

You will identify needs, support access to services and build networks with community groups, housing, finances, stress and other determinants of health. A driving licence is essential to visit patients and communities.

Qualifications

• Holds a recognised healthcare or advisory qualification (or equivalent relevant experience).
• Good standard of education with excellent literacy and numeracy skills.
• Leadership and/or management qualification (or equivalent relevant experience)

Responsibilities

• Take referrals from a wide range of agencies within primary care networks and services.
• Provide personalised support to individuals and carers to improve wellbeing.
• Develop personalised support plans and connect people to community groups and services.
• Promote social prescribing and build links with local agencies to enable referrals.
