Job summary

Sunderland GP Alliance is seeking a Social Prescribing Link Worker in Sunderland to support individuals in managing health and wellbeing. You will engage with patients, families and carers, building trust and delivering personalised support that focuses on what matters to them.

The role involves multi-disciplinary teamwork within PCN settings, referrals, care planning and connecting people to community resources to improve health outcomes.

Qualifications

• Ability to identify risk to self and others, safeguarding incidents.
• Experience of working with voluntary organisations or groups.
• Experience of collecting and recording information and data.

Responsibilities

• Take referrals from a wide range of agencies under guidance of referring GP.
• Provide personalised support to empower individuals and families, focusing on what matters to them.
• Develop personalised care plans and connect people to community groups and services.
• Manage and prioritise own caseload and maintain awareness of when to refer to other health professionals.
