Job summary

One Wight Health Ltd is seeking a full-time Social Prescribing Link Worker to be employed by One Wight Health Ltd and to work within the Islands South Primary Care Network at the Bay Medical Practice, covering Sandown and Shanklin branches.

You will identify patient needs, support access to services and build networks to connect people with housing, finances, stress relief, loneliness or healthy lifestyle options.

Qualifications

• Holds a recognised healthcare or advisory qualification (or equivalent).
• Ability to demonstrate management of own caseload and prioritisation.
• Experience providing advisory services for complex patient situations with multi-agency working.

Responsibilities

• Take referrals from GP practices, hospitals, fire, police, and VCSE organisations.
• Provide personalised support to individuals and carers to improve health and wellbeing.
• Develop personalised support plans and connect to community services.
• Work with local partners to foster sustainable VCSE organisations and community assets.
