Job summary

Sunderland GP Alliance is seeking a proactive Social Prescribing Link Worker to support people in Sunderland to take control of their health and wellbeing. You will work with GP practices, PCNs and VCSE organisations to connect people to community services and co-produce personalised care plans.

The role requires travelling across Sunderland, engaging diverse communities, and maintaining safeguarding and data protection standards while supporting individuals and families.

Qualifications

• Experience in social prescribing and working with GP practices.
• Ability to travel across Sunderland and surrounding areas.
• Awareness of data protection and safeguarding practices.
• Strong communication and collaboration with diverse communities.

Responsibilities

• Provide personalised support to empower individuals to take control of health and wellbeing.
• Co-produce a simple personalised care and support plan.
• Develop links with local VCSE organisations to enable social prescribing referrals.
• Support referrals and manage caseload within the PCN MDT context.
• Educate staff about community services and how to access them.
