Highly Specialised Occupational Therapist

Merton Domiciliary Therapies and Merton Holistic Assessment and Rapid Intervention Team are multidisciplinary teams providing high-quality rehabilitation to the Merton population.

The post holder will support adults living with frailty and complex health needs to optimise their independence, health, and wellbeing, and prevent hospital admissions or premature moves to care homes. We are committed to the assessment and rehabilitation of patients living within the Merton community.

The post holder plays a key role within the team, working with patients, service users, and their carers to provide highly specialist occupational therapy assessment and treatment plans to help individuals maintain or regain mobility, independence, and wellbeing at home. This includes setting goals and reviewing outcomes to reduce the need for long-term support.

The post holder will provide local clinical leadership and advice for occupational therapists and the multidisciplinary team in domiciliary therapies. They will also be responsible for the education and supervision of junior occupational therapy staff, rehabilitation assistants, and OT students.

Responsibilities include:

  1. Providing a highly specialist occupational therapy service as part of a multidisciplinary team, to patients in their homes and clinic settings.
  2. Delivering clinically effective, person-centred, and evidence-based occupational therapy assessments and interventions for patients with complex physical health needs.
  3. Assisting the Clinical Lead in planning and delivering occupational therapy services, complying with RCOT standards.
  4. Supporting and supervising junior staff.
  5. Participating in service development, management, research, and project responsibilities.
  6. Providing training on occupational therapy interventions to staff.
  7. Adhering to the College of Occupational Therapists Code of Ethics and Professional Conduct.
  8. Leading the service in the absence of the Clinical Lead.
  9. Leading clinical triage alongside other senior therapists and the Clinical Lead.
  10. Managing standard operating procedures related to policies and processes.
  11. Travelling to multiple sites as required.
