Merton Domiciliary Therapies and Merton Holistic Assessment and Rapid Intervention Team are multidisciplinary teams providing high-quality rehabilitation to the Merton population.
The post holder will support adults living with frailty and complex health needs to optimise their independence, health, and wellbeing, and prevent hospital admissions or premature moves to care homes. We are committed to the assessment and rehabilitation of patients living within the Merton community.
The post holder plays a key role within the team, working with patients, service users, and their carers to provide highly specialist occupational therapy assessment and treatment plans to help individuals maintain or regain mobility, independence, and wellbeing at home. This includes setting goals and reviewing outcomes to reduce the need for long-term support.
The post holder will provide local clinical leadership and advice for occupational therapists and the multidisciplinary team in domiciliary therapies. They will also be responsible for the education and supervision of junior occupational therapy staff, rehabilitation assistants, and OT students.
