Healthcare Support Worker - Integrated Neighbourhood Team (Wound Healing Focus)
An exciting opportunity has arisen for an enthusiastic and motivated Healthcare Support Worker to join the Integrated Neighbourhood Team (INT) at University Hospitals Coventry and Warwickshire NHS Trust.
Working as part of a multidisciplinary neighbourhood team, you will support the delivery of high-quality, person-centred care to patients in their own homes, care homes and community clinics. You will work alongside Registered Nurses and other healthcare professionals to help patients maintain their independence, manage long-term conditions and achieve the best possible health outcomes.
A key element of this role will be supporting patients with wound healing needs, including lower limb ulceration, surgical wounds, pressure ulcer prevention and other complex wounds. Under the direction of Registered Nurses, you will contribute to the delivery of treatment plans, monitor progress and support patients in achieving positive healing outcomes.
Whilst wound healing will be a priority area of practice, the role also involves supporting the wider needs of patients through delegated aspects of their care plan. This may include tasks such as medication administration, insulin administration, clinical observations and other planned interventions, ensuring patients receive holistic, coordinated care during each visit.
Our Integrated Neighbourhood Teams provide coordinated care closer to home, bringing together a range of health and social care professionals to meet the needs of local populations.
The team supports patients with acute, chronic and complex health needs, helping to prevent avoidable hospital admissions and enabling people to remain safe and independent within their own communities. Working collaboratively across services, we aim to deliver proactive, responsive and personalised care.
As a Healthcare Assistant within the team, you will play a vital role in supporting patients and contributing to the delivery of safe, effective and compassionate care.
