Qualifications

• NMC registered Health Visitor with current practising credentials.
• Proven ability to communicate clearly with families and colleagues.

Responsibilities

• Lead delivery of the Healthy Child Programme across the 6 High Impact Areas.
• Develop personalised care plans through effective partnerships with families and professionals.
• Apply evidence-based practice to improve outcomes for children, young people and families.
