Job summary

HCRG Care Group is seeking a Specialist Community Public Health Nurse (Health Visitor) to lead the 0-5 Healthy Child Programme in a defined Wiltshire locality. You will assess children and families, plan personalised care, and work with partner agencies to reduce health inequalities.

The role requires SCPHN Health Visiting qualification, current NMC registration, and experience in Health Visiting teams. The position offers a 30-hour week at Band 6 AfC salary with NHS Pension and development

Qualifications

• Current NMC registration is required.
• Experience leading Health Visiting teams is essential.

Responsibilities

• Lead delivery of the 0-5 Healthy Child Programme in a defined locality.
• Assess health and wellbeing needs of children and families.
• Address safeguarding concerns and promote early intervention.
• Build strong relationships with families and multi-agency partners.
• Manage caseload and evaluate care and outcomes.
