Job summary

Derbyshire Healthcare NHS Foundation Trust is seeking experienced Health Visitors to join the Children's Universal Services in Derby City. The post focuses on delivering the Healthy Child Programme with a defined caseload and collaborative work with partner agencies.

Applicants should hold SCPHN and SSP or equivalent, with strong leadership and communication skills to support development of staff and junior colleagues. Flexible working and professional development are encouraged.

We are looking for skilled professional Health Visitors who have excellent communication skills and who can deliver compassionate care to support the service, making a real difference to improve the health outcomes for children and families in Derby City.

You will have experience of working with diverse communities and dealing with highly emotive situations, be able to engage with families and build trusting professional relationship. Putting children and young people at the heart of everything you do, you will be passionate about working collaboratively with our partner agencies and will embrace supporting the development of students and new team members.

We are an innovative service providing health visiting services to the families of Derby and we currently have some great opportunities for you to join our team.

Main duties of the job

You will be responsible for the delivery of health visiting services to a defined corporate caseload within Derby City, working within the 0-19 framework ensuring a seamless service to the family as appropriate and working collaboratively with partner agencies.

You will lead on the delivery of the Healthy Child Programme as defined within the service specification, identifying packages of care based on the Universal, Targeted and Specialist models of service delivery. You will provide leadership and supervision to other members of the team and work within Derbyshire Healthcare NHS Foundation Trust policies and procedures.

Detailed job description and main responsibilities

• Provide quality evidence-based PH Nursing service (Health Visiting).
• Undertake regular training and have clinical assessmentsin line with Baby Friendly Accreditation.
• Lead on the delivery of the Healthy Child Programme.
• Excellent communication skills both verbal and written.
• Lead the skill mix team.
• Analyse and identify key areas of need within the local community.
• Work in partnership with key agencies.
• Write reports, collect data, and provide statistical returns as required by the Trust
