Job summary

Joined Up Care Derbyshire is seeking a Health Visiting specialist to deliver the Healthy Child Programme to families and manage a defined community caseload. You will lead safeguarding, assess care needs and develop, implement and evaluate programmes of care, applying evidence-based standards.

You will prescribe within the nurse prescribing formulary, mentor staff, and work across Derbyshire with flexible training days and team collaboration. A dynamic, travel‑driven role in community health.

Qualifications

• SCPHN qualification in Health Visiting with V100/V150 entitlements.
• NMC registration (RN/RM) to practice and prescribe as required.
• Experience in mentoring students and leading practice developments.

Responsibilities

• Provide Health Visiting services to families and deliver the Healthy Child Programme.
• Manage a defined active caseload and address Safeguarding Children issues.
• Assess needs, develop and evaluate programmes of care, and implement evidence-based standards.
• Prescribe from the Nurse Prescribing Formulary as appropriate.
• Lead and manage a health visiting team and coordinate local services.

Job description

• To provide Health Visiting services to families in the community team caseload by delivering the Healthy Child Programme.
• To manage a defined active caseload and take responsibility for Safeguarding Children issues within that caseload.
• To have continuing responsibility for the assessment of individuals needs and the development, implementation and evaluation of programmes of care.
• To prescribe according to assessed individual need from the Nurse Prescribing Formulary.
• To manage a team of Health Visiting support staff.
• To manage and take continuing responsibility for a defined caseload/geographical area and be responsible for the assessment of care needs, the development, implementation and evaluation of programmes of care and setting of defined standards of care which are evidence based and meet Trust Policies and Procedures to ensure quality of care for patients.
• Identify, assess and determine client need and where appropriate prescribe from within the nurse prescribing formulary in accordance with evidence, practice-based, patient medical history and Trust Policies and Procedures.
  The right candidate will hold the SCPHN qualification in Health Visiting and the V100/V150 and be an innovative and dynamic team player who is looking for a new challenge.
• The base for this post is in Repton, this location and requires travelling across the locality, therefore own transport is essential. Training days and meetings are sometimes in other parts of Derbyshire.
