Job summary

Derbyshire Community Health Services NHS Foundation Trust seeks Health Visitors to deliver the Healthy Child Programme across the North High Peak area. The role requires SCPHN Health Visiting and V100/V150 nurse prescribing qualifications, with travel across a rural locality and occasional meetings in Derbyshire.

You will manage caseloads, address safeguarding, assess needs, and develop care programmes while supporting the local 0-19 team that has Gold accreditation.

We are seeking to appoint dedicated and motivated Health Visitors to work within the High Peaks, North Dales and Glossop locality. The High Peaks, North Dales and Glossop 0-19 Team have been awarded Gold accreditation for their work with Families and Communities.

DCHS is a forward-thinking, supportive Trust, who are a UNICEF Baby-Friendly accredited organisation and achieved Outstanding CQC status in 2019. Working in a rural community you will find that there are lots of opportunities for developing public health work with communities and partner agencies, as well as delivering the core contacts within the Healthy Child Programme.

The right candidates will hold the SCPHN qualification in Health Visiting and the V100/V150 and be innovative and dynamic team players who are looking for a new challenge.

The base for this post is in New Mills however the team covers the whole of the North High Peak area. This is a rural location and requires travelling across the locality. Training days and meetings are sometimes in other parts of Derbyshire.

  • To provide Health Visiting services to families in the community team caseload by delivering the Healthy Child Programme.
  • To manage a defined active caseload and take responsibility for Safeguarding Children issues within that caseload.
  • To have continuing responsibility for the assessment of individuals needs and the development, implementation and evaluation of programmes of care.
  • To prescribe according to assessed individual need from the Nurse Prescribing Formulary.
  • To manage a team of Health Visiting support staff.

  • To manage and take continuing responsibility for a defined caseload/geographical area and be responsible for the assessment of care needs, the development, implementation and evaluation of programmes of care and setting of defined standards of care which are evidence based and meet Trust Policies and Procedures to ensure quality of care for patients.
  • Identify, assess and determine client need and where appropriate prescribe from within the nurse prescribing formulary in accordance with evidence, practice-based, patient medical history and Trust Policies and Procedures.
