Derbyshire Community Health Services NHS Foundation Trust seeks Health Visitors to deliver the Healthy Child Programme across the North High Peak area. The role requires SCPHN Health Visiting and V100/V150 nurse prescribing qualifications, with travel across a rural locality and occasional meetings in Derbyshire.
You will manage caseloads, address safeguarding, assess needs, and develop care programmes while supporting the local 0-19 team that has Gold accreditation.
We are seeking to appoint dedicated and motivated Health Visitors to work within the High Peaks, North Dales and Glossop locality. The High Peaks, North Dales and Glossop 0-19 Team have been awarded Gold accreditation for their work with Families and Communities.

The base for this post is in New Mills however the team covers the whole of the North High Peak area. This is a rural location and requires travelling across the locality. Training days and meetings are sometimes in other parts of Derbyshire.
