Job summary

Derbyshire Community Health Services Nhs Foundation Trust is seeking a Health Visitor to work across the South Derbyshire and Derbyshire Dales locality. You will deliver the Healthy Child Programme, manage caseloads, and lead Health Visiting staff. SCPHN Health Visiting qualification and V100/V150 Nurse Prescribing are required, with UK registration.

The post requires travel across the locality and uses NHS safeguarding and public health practice throughout the role.

Qualifications

• SCPHN Health Visiting qualification or equivalent.
• Current UK professional registration is required.
• Experience in public health and mentoring students in practice is essential.

Responsibilities

• Provide Health Visiting services to families delivering the Healthy Child Programme.
• Manage a defined caseload and oversee safeguarding within the caseload.
• Assess care needs, develop and evaluate programmes of care.
• Prescribe from the Nurse Prescribing Formulary where appropriate.
• Lead and coordinate a team of Health Visiting support staff.

Job summary

We are seeking to appoint a dedicated and motivated Health Visitor to work within the South Derbyshire and Derbyshire Dales locality.

The South Derbyshire and Derbyshire Dales 0-19 Team have been awarded Gold accreditation for their work with Families and Communities.

The right candidate will hold the SCPHN qualification in Health Visiting and the V100/V150 and be an innovative and dynamic team player who is looking for a new challenge.

The base for this post is in Repton, this location and requires travelling across the locality, therefore own transport is essential. Training days and meetings are sometimes in other parts of Derbyshire.

Main duties of the job

• To provide Health Visiting services to families in the community team caseload by delivering the Healthy Child Programme.
• To manage a defined active caseload and take responsibility for Safeguarding Children issues within that caseload.
• To have continuing responsibility for the assessment of individuals needs and the development, implementation and evaluation of programmes of care.
• To prescribe according to assessed individual need from the Nurse Prescribing Formulary.
• To manage a team of Health Visiting support staff.

Job responsibilities

• To manage and take continuing responsibility for a defined caseload/geographical area and be responsible for the assessment of care needs, the development, implementation and evaluation of programmes of care and setting of defined standards of care which are evidence based and meet Trust Policies and Procedures to ensure quality of care for patients.
• Identify, assess and determine client need and where appropriate prescribe from within the nurse prescribing formulary in accordance with evidence, practice-based, patient medical history and Trust Policies and Procedures.
• RN or RM (NMC registered)
• SCPHN Qualification - Degree, Diploma or Cert
• Nurse Prescribing
• Specialist Community Public Health Nurse Post registration study
