Job Description:

GLL is seeking a flexible Health Referral Facilitator to work across the sport and leisure facilities at Atherton Leisure Centre. The role involves prescribing suitable physical activity and supporting behaviour change for Medically Referred clients, including setting up and conducting group and individual training sessions, managing referral processes, and maintaining accurate records and data.

This position specifically involves delivering a Falls Prevention community-based programme and will require travel to community venues and Leisure Centres across Newham. A Postural Stability Instructor (PSI) qualification is desirable but not essential; training will be provided to the successful candidate.

The Health Referral Facilitator reports to the Healthwise Coordinator / Health Manager.

Key Responsibilities:

1. Ensure the smooth operation of the Physical Activity Referral Scheme (PARS), optimizing resource use.
2. Conduct health and lifestyle assessments and develop safe, appropriate physical activity/exercise programmes.
3. Act as the primary contact for patients referred to the scheme.
4. Provide evidence-based, culturally sensitive health and lifestyle information.
5. Collaborate effectively within the operational team, wider health teams, and GLL support services.

Qualifications and Experience Needed:

• A recognised Level 3 Fitness Instructor or Personal Training qualification.
• Experience in pre-screening and assessing patients for suitability for physical activity.
• Experience in monitoring and progressing individuals through exercise programmes.
• Understanding of the health and fitness industry.
• Ability to motivate and support behaviour change.
• Excellent time management and punctuality.
