Job description

GLL is seeking a Health Referral Facilitator to work across sport and leisure facilities in Croydon. If you possess the necessary skills and ambition, this is a great opportunity to advance your career in the health and leisure industry.

Role Overview

The Health Referral Facilitator will prescribe appropriate physical activity and support behavior change for Medically Referred clients. Responsibilities include setting up and conducting group and individual training sessions, managing the GP Referral program, referral processing, record keeping, and data management.

Reporting To

The role reports to the Healthwise Coordinator / Health Manager.

Key Responsibilities

1. Ensure the smooth operation of the Physical Activity Referral Scheme (PARS), maximizing resource effectiveness.
2. Conduct health and lifestyle assessments with referred patients and develop safe, suitable exercise programs.
3. Serve as the first point of contact for patients referred to the scheme.
4. Provide culturally sensitive, evidence-based information on healthy lifestyles.
5. Collaborate effectively with team members, health professionals, and support services.

Candidate Requirements

• Recognized Level 3 Fitness Instructor or Personal Training qualification.
• Experience in pre-screening and assessing patients for physical activity suitability.
