City Community Team is seeking a part-time Health Care Support Worker based at Five Rivers Health and Wellbeing Centre. We provide complex care in the community setting, caring for vulnerable individuals often in challenging environments.
We aim to deliver care closer to home, utilizing available resources to meet local needs.
The role is demanding but rewarding. We seek an enthusiastic, motivated, and hardworking individual who can work flexibly and efficiently.
City Community Team cares for patients registered with a South Wiltshire GP. You will work alongside multidisciplinary team members such as Occupational Therapists, Physiotherapists, Assistant Practitioners, and other specialists including Consultant and Advanced Clinical Practitioners. Our shift patterns include 7-3, 8.30-4.30, and 2-10pm.
