Job description

This position is open to staff working at the Trust in the 'ECT Family' this includes currentEast Cheshire NHS Trust employees,both NHSP and ECT bank staff, ISS staff and volunteers at the Trust. Unfortunately, we cannot accept applications from applicants not listed within these groups., We have several positions available which will involve shift work including days, nights and weekends. Our office is based in Congleton, but the care packages are based within East Cheshire so the ability to travel is essential part of this role, please demonstrate on your application form how you would meet this requirement. The post-holder will be responsible for the holistic care of a children and young adults with complex health care needs and/or technology dependence in the Clients own home. Following a period of competency based training and supervision; HCA's will be expected to carry out all relevant forms of care following client care plans and East Cheshire Trust policy, procedures and guidelines. The post- holder will be working in conjunction with the family, other carers, health care professionals' and teachers to ensure a holistic approach to the child / young person's care. Qualifications

• Good standard of general education or ability to demonstrate equivalent.

Desirable

• GCSE (Grade C pass or Level 2 skills Math/English), Knowledge, skills or experience in health/social care
• Shows insight into the needs and circumstances of patient group.
• Ability to recognise limitations of the role.
• Understand the concept of self-care and self-management principles
• Knowledge of clinical procedures, observations and care.
• Ensures that own actions support equality, diversity, rights and maintain quality.
• Ability to follow direction during a period of supervision and training.

• Awareness of how chronic illness affects the child and family

• Previous Complex Care experience
• Knowledge of Microsoft office systems
• A basic understanding of the need to follow care plans in order to work within the boundaries of care, and follow guidelines and procedure.
