Job summary

East Cheshire NHS Trust is seeking a Health Care Assistant to deliver holistic care to children and young people with complex health needs at home and in education settings.

After competency-based training, you will follow client care plans in partnership with families and professionals, supporting continuity and quality of care across services.

Strong communication, literacy, and teamwork are essential, with a commitment to equality and safeguarding as part of a diverse workforce.

Qualifications

• Good general education or equivalent demonstrated.
• Care Certificate (or commitment to undertake on commencement).
• NVQ Level 3/City & Guilds - health/social care related subject, or equivalent experience.

Responsibilities

• Provide holistic care to children and young people in their own home.
• Work with families, carers and professionals to follow care plans.
• Attend competency-based training and supervision.
• Support delivery of care in education settings as needed.
