Job summary

Cardiff and Vale University Health Board is recruiting Senior Health Care Assistants to provide high-quality care in patients' homes across East, North, and West Hubs. The role requires independence, reliability, and willingness to travel to different sites as part of a busy community nurse team.

Qualifications

  • Level 3 vocational qualification in health or social care or equivalent experience.
  • L2 Functional/Key skills or equivalent clinical experience in community health care.
  • GCSE English and Maths at grade C or equivalent.
  • Experience working in a multidisciplinary health/social care team and strong communication.
  • Willingness to develop IT skills and use electronic records systems.

Responsibilities

  • Provide high-quality nursing care in patients' homes as part of a community nurse team.
  • Cross-cover for other teams and travel to work from different sites as needed.

Job description

We have nursing teams based in the following sites:


  • North Hub - based at Haxby Health Centre

  • Selby/South Hub based Selby Hospital, Tadcaster Health Centre or Sherburn in Elmet

  • East Hub - Tang Hall Clinic

  • West Hub - South Hambleton and North Ryedale


This is a community post and you will work independently in patients' homes. It is essential that you the ability to transport yourself and equipment across the locality during the course of a normal working day. Travel expenses will be covered by expenses and parking at our community sites is free for clinical staff. We require staff to cross cover for other teams and potentially travel to work from other sites. We currently have positions available in the East Hub which would be the successful candidate's main base. We want to recruit caring and motivated Senior Health Care Assistants to provide the best possible care to our patients. Previous experience of caring for people at home and a commitment to providing care to a high standard is essential. You will have the willingness to extend and develop your skills, and have the ability to apply your knowledge and skills to practice. The successful candidate will be required to work over a 7 day period, and will work as an integral part of a busy forward thinking and innovative Community Nurse Team.




  • Level 3 vocational qualification in health or social care, or willingness to undertake or equivalent experience in community health care

  • L2 Functional/Key skills OR equivalent clinical experience in a community setting, Minimum of C grade GCSE or equivalent in English and Maths, Previous experience working and communicating with the public in a health/social care/educational setting.

  • Understanding of the role of a Senior Health Care Assistant, Experience of working in a multidisciplinary team in health/social care or education, Basic IT skills

  • Demonstrate effective communication skills

  • Able to work independently without direct supervision

  • Ability to manage, prioritise and organise own workload with advice and consultation, Advanced IT skills, ECDL qualification or electronic patient records e.g. using S1/EMIS/CPD
