This is an exciting opportunity for an experienced Nursing Associate to work as part of our friendly and dynamic multi-disciplinary care team. You must be an enthusiastic individual with excellent Communication, organisational skills and motivation in your approach to care delivery to ensure we maintain a high-quality service for our patients in the changing environment of community services. Care Home practitioners (CHP) work as part of the wider health and social care team and have direct contact with patients within the Residential home setting, providing high quality and compassionate care. CHP work at a level above that of Healthcare Support Workers and have a more in depth understanding regarding patient care. CHP provide training and support to care homes in line with current policies and guidelines. CHP work closely with their respective GP surgery linked to the residential homes and other Health and social services.
