Honiton Surgery in Honiton, Devon, is recruiting a Pharmacist to join the practice team. You will work with GPs to optimise medicines for long-term conditions, conduct prescribing reviews and support the PCN with pharmacotherapy initiatives.

The role offers both full-time and flexible (0.6 WTE) working patterns and involves collaboration with the PCN pharmacists and technicians to improve patient care and safety.

Formación

• Must be a registered Pharmacist with the General Pharmaceutical Council (GPhC).
• Experience in general practice medicine optimisation and prescribing would be advantageous.
• Demonstrates excellent communication and teamwork abilities.

Responsabilidades

• Carry out medicine optimisation and review plans for Long Term Conditions.
• Perform structured medication reviews for priority patients via face-to-face and phone appointments.
• Liaise with secondary care and community services to ensure safe medicines use.
