Key Duties
Providing expert services and support to patients registered with our HubOffering clear and accessible advice on medicines, repeat prescriptions, and general pharmaceutical queriesAssisting with prescribing, medicines optimization, and high-risk drug management audits across primary care sitesUsing critical thinking and effective communication skills to assess and address patient needsPrescribing urgent prescriptions and ensuring accurate re-prescribing using summary care record informationIdentifying and addressing clinical risks in line with professional responsibilitiesKeeping clear, accurate, and timely records of all consultations and advice givenActively engaging with colleagues to deliver outstanding service
