We are looking for a Clinical Pharmacist, to work across Cordwallis Rd and Woodlands Park GP Surgeries. We are looking for a Clinical Pharmacist who acts withintheir professional boundaries, supporting and working alongside a team ofpharmacists in general practice. In thisrole they will be supported by a Senior Clinician and Line Manager who willdevelop, manage and mentor them.
Required 30 hrs per week.

Main duties of the job
- See (where appropriate) patients with single or multiplemedical problems where medicine optimisation is required (e.g. COPD, asthma).
- Undertake structured medication reviews with patients andproduce recommendations for agreement on prescribing changes and monitoring.
- Provide patient facing clinics for those with questions,queries and concerns about their medicines in practices.
- Review the use of medicines most commonly associated withunplanned hospital admissions and readmissions through audit and individualpatient reviews.
- Reconcile medicines following discharge from hospitals,intermediate care and into care homes, including identifying and rectifyingunexplained changes and working with patients and community pharmacists.
- Signposting to other services and/or healthcareprofessionals where appropriate, while working within scope of practice andlimits of competency.
- To manage repeat prescription requests, resolving querieswhere possible within scope of practice.
- To maintain full and complete records of all patientcontacts using appropriate clinical templates and coding .
- Provide independent information to patients with regard tomedicines and prescribing changes, initiating further support fromother healthcare professionals where appropriate.
- The post holder will be supported to develop their role tobecome a non medical prescriber.
- Work with practices to ensure full compliance with CareQuality Commission standards for safe and effective care.

Qualifications
- Mandatory registration with General Pharmaceutical Council Masters degree in pharmacy (MPharm) or equivalent.
- Completed either the PCN CPPE or IP course.
- Membership of the Royal Pharmaceutical Society A member of or working towards Faculty membership of the Royal Pharmaceutical Society Specialist knowledge acquired through postgraduate diploma level or equivalent training/experience Independent prescriber or working towards/intent of gaining independent prescribing qualification.
Experience
- Minimum of 2 years post qualification experience.
- Experience working in a GP practice.
- Experience of working in primary care.
