A leading healthcare provider in Hounslow seeks a Clinical Pharmacist IP to enhance patient care through medication optimization. The role involves collaboration with GPs and multidisciplinary teams, ensuring effective prescribing and patient management. Ideal candidates will have a Pharmacy degree and independent prescribing qualification, along with experience in a GP practice.
The position requires a qualified pharmacist with an independent prescribing qualification, who will operate within their professional limits as a member of a multidisciplinary team that includes GPs, ANPs, Physician Associates, Paramedics, and First Contact Physiotherapists.
The successful candidate will collaborate closely with the GP and other practice clinical and non clinical staff, playing a key role in supporting GPs and the broader primary care team to provide safe and effective care. They will provide medication-related patient services, including direct patient care. As a leader, the candidate will guide and support the practice team to ensure safe and effective prescribing, while maintaining and enhancing the quality of patient care within the practice.
Hounslow Family Practice is a partnership of two GPs, providing care for 6,100 patients in Hounslow. Our exceptional clinical team includes 2 GP Partners, 2 Salaried GPs, a Clinical Pharmacist, 1 Practice Nurse, 1 Physician Associate, and 2 Health Care Assistants. This clinical team is supported by an administrative staff, which includes a Practice Manager, an Administration team, and a Reception team, all dedicated to serving our patient population.
Consult with patients who have one or more medical conditions that require medication optimization (e.g., COPD, asthma, hypertension).
Review the ongoing necessity of each medication, assess monitoring needs, and provide support to patients in managing their medications, ensuring they achieve the best outcomes from their treatments (i.e., medication optimization).
Implement changes to medications in response to MHRA alerts, product withdrawals, and other local and national guidelines.
