Job summary

Hampton Medical Centre in Macclesfield is seeking a Clinical Coder to review hospital letters, discharge summaries and diagnostic reports, and record them in SNOMED CT on the clinical system. The post holder must adhere to confidentiality and practice protocols.

You will work with GPs and the wider team, identify missing information, support data validation and audits, and ensure coding accuracy. Training in EMIS Web or SystmOne is expected.

Qualifications

• Experience in clinical coding, medical records or healthcare administration.
• Strong attention to detail and accuracy.
• Familiarity with SNOMED CT coding.
• Ability to learn EMIS Web or SystmOne.

Responsibilities

• Review clinical correspondence, including hospital letters, discharge summaries, diagnostic reports and referrals.
• Code using SNOMED CT and update patient records in line with practice protocols.
• Ensure records are accurate, complete and confidential, identifying duplicates or incorrect entries.
• Identify missing or unclear information and refer queries to the appropriate clinician.
• Support data validation, audits, disease registers and correction of coding discrepancies.
• Process clinical documents and coding worklists within agreed timescales.
• Prioritise urgent or clinically significant information and priority concerns promptly.
• Ensure documents are correctly coded, filed and workflowed, directing items to the right team.
