Remote clinical coder post with General Practice Medical Solutions, working across multiple GP surgeries in England. The work is reviewing patient records and incoming clinical correspondence, then coding diagnoses, procedures and investigations accurately in the GP system using SNOMED CT. The post is part-time and can be done from home, and it means adapting to each practice's workflows and systems. Prior primary care coding experience and working knowledge of SNOMED CT decide fit.
What the job involves
- Code diagnoses, symptoms, procedures, investigations and treatments from patient records, reflecting the source documents without unsupported diagnoses
- Review consultations, discharge summaries, outpatient letters, pathology results, referrals and other incoming clinical correspondence to identify information to code
- Apply SNOMED CT codes and terminology in line with NHS and primary care standards
- Support practices to keep patient records accurate, complete and up to date, including long-term conditions and significant diagnoses
- Identify gaps in coding, duplicate or contradictory codes, and discrepancies between free text and coded data, then amend or clarify existing codes
- Escalate clinical queries and uncertainties to an appropriate clinician rather than making clinical judgements
- Flag patients who may need further clinical review where the record indicates a potential issue
- Meet agreed coding accuracy, productivity and turnaround standards while managing competing priorities across practices
- Follow each practice's protocols and workflows and keep clear communication with practice-based contacts
- Take part in quality assurance, audit and sample checks, and act on feedback
- Support coding-related data-quality projects and improvement initiatives
- Keep records of work completed where required and stay aware of changes to NHS primary care coding requirements
